Community Paramedic Program

TBlood Pressure Cuffhe Pelham Community Paramedic Program was created to assist residents of Pelham by providing a personal connection to social and medical resources. The Pelham Community Paramedic Program provides these services at no charge, to better enhance and enrich the lives of Pelham residents. It's care beyond the call.

The program operates in collaboration with our local area agency on aging (M4A). We can assist with or connect residents to the following resources:

  • Dementia and Alzheimer's Caregiver Assistance
  • Chronic Disease Self-Management Education (One-on-one sessions and workshops)
  • Senior Services or Disability Resources
  • Mental Health or Substance Abuse
  • Medication Therapy Management
  • Medicare and Medicaid Counseling
  • Home Safety Assessments / Fall Reduction
  • Overcoming Barriers to Healthcare

Who's Eligible?

Anyone who lives in Pelham is eligible to participate in the Community Paramedic Program. While the program's main focus is on providing help to those with Alzheimer's and Dementia, the program also serves people with special needs, those who may have a physical disability, and people who find themselves in a situation where they've become someone's primary caregiver.

What to Expect

Once you have completed and submitted the form to enroll in the Community Paramedic Program, one of our Community Paramedics will reach out to you via phone. The two of you will then talk about your current situation to better understand your needs. Once that initial phone call takes place, an in-person meeting will be scheduled for an initial assessment. This is when our Community Paramedics will get to know you learn more about your medical history in order to create an individualized plan and assessment schedule.
